Year End Evaluation

In paragraph form, using complete sentences, answer the following questions. The paragraphs must be prepared and typed on a separate piece of paper.

1. What did you like about this year? (You may include descriptions of special trips or activities in which you were involved.)

2. Based on your experiences this year, what advice would you give to an incoming student?

3. Adult Character Reference: Name an adult, other than a family member, who made a difference for you this year. Give a brief explanation as to why and how this person made a difference. Include an address and phone number if possible for future reference.

4. Restate the goal you set at the beginning of the year. Explain the steps you took to achieve your goal. If you did not achieve your goal, explain why.
