Parent/Student/Teacher Liability Release

I, _______________________________________, have read the course overview and I understand the “hands – on” nature of this course. I understand that I may have to touch and be touched by other students to complete this course, and that this is a learning environment. Students will be using sharp objects such as scissors/tweezers, practicing CPR, and using adhesive tape on each other. Because of these activities, this course requires students to follow instructions and safety guidelines described by the instructor during class to avoid injury while learning. All efforts will be made both by the instructor and other students to prevent any injury or bodily harm to all students during class. Students that have ANY allergies must report them to the instructor within the first week of class, especially allergies to latex, adhesives, etc. Failure to comply with safety guidelines given by the instructor will result in disciplinary consequences that will fit the infraction.

I have read and understand everything and that I am responsible for my own actions, and that all efforts must be made to avoid injury during this course. I will report any breech of safety to the instructor immediately.
